Yes I have. Back in the day I use to do some search engine optimization where I would use three different computers in my household type in the same keywords at the same time (using a macro) and google would periodically make me fill out a captcha.

Don't tell us that! If you remember which one you did it on, go to that HIT on your list and send a message to the requester for that hit. I know I have messed up a hit here myself. I sent a message apologizing and a lot of times they will say it's ok, so long as your other work is good.
Yeah took care of the contact to x8 minutes after typing that. I'll take a reject since I clearly didn't follow instructions. I have submitted quality work to x8 previously so I'm sure they'll factor that in as well.
