I remember once reading about someone who did have this as a real problem. They were born without genitalia of any kind. Just a pee-hole, like a plastic baby doll. No inner or exterior sex organs. He (and or she) took a kind of pleasure in being different, refusing to tick either box when asked to identify if male or female. Seeing as it was the only kind of sexual pleasure that would ever be available to them, it seems kind of cruel to refuse to grant it. That said, I do understand what you're saying. The vast majority of these people are just being pretentious assholes.
Yeah I was assuming that they weren't one of the .000001% that might be born without genitalia. There's also usually no choice for hermaphrodite which is more common but I doubt that it's a complaint someone would have if they were legitimately one.
