Trusted Worker Request Form

Oscar sent me a link to a Trusted Worker Request form which asks for my MTurk ID, Name, and e-mail address. Has anyone received this and filled this out? Is it safe to list your name?

 

I did. He said me to wait 3 more months even if I did like 2000 his hits and had 90 quality -,-

 

After several weeks with 90 or above my qualification finally went back down to 88.

 

Oscar...is this the one with those business card hits?
